Timpanogos Tribe Downloads Page

The Timpanogos Tribe membership application is a free download for people believing they have Timpanogos Blood and qualify for membership. Applications should be returned to the Timpanogos Tribe P.O. Box 327 Ft. Duchesne, Utah 84026, all applications are required to have birth/death certificate of each individual named on the lineage chart including the person applying. All applications must be signed.

Download Instructions:

1. Click on "ApplicationDocuments" below here.

2. Locate the downloaded file on your computer named "TIMPANOGOS NATION - Membership Application.zip."

3. Double click on the file to unzip the contents.

4. When the file is unzipped you will see nine files. Click on each file to open, then print.
